All contestants must complete the entry form and return by DECEMBER 1st, 2007. Entries postmarked, e-mailed or faxed after this date will not be considered and will be put on a mailing list for the following year. “BATTLE OF THE BANDS” is open to BANDS ONLY, 13-19 years of age.

By entering, each contestant agrees to abide by all rules and regulations of the competition and that all decisions by the South Florida Fair, its representatives and judges are final.

**TALENT UNDER THE AGE OF 18 - Parent or legal guardian must give written permission by filling out and checking the "I accept" button below as digital signature. If there is more than one individual under the age of 18 performing, a separate form must be filled out and signed for each individual TALENT. This form may be duplicated, and must be received by DECEMBER 1st, 2007.
